In medicine, measurement errors can have life-altering — or life-ending — consequences. A wrong unit, a misplaced decimal, or a confused abbreviation can mean a patient receives ten times the intended dose. Understanding the units used in healthcare isn't just educational; it's a matter of safety. This article explains the most important medical measurement units, how they are used, and where confusion is most likely to arise.

The Core Units of Medical Measurement

Healthcare uses a specific subset of metric and non-metric units. Unlike everyday measurement, medical units must be interpreted with extreme precision — and the difference between adjacent units on the scale is often 1,000-fold.

Mass Units: Grams, Milligrams, and Micrograms

Most drug doses are expressed in one of three mass units:

  • Gram (g) — the base unit. Most drugs are too potent to be dosed in grams; a full gram of many medications would be lethal.
  • Milligram (mg) — 1/1,000 of a gram. The most common unit for tablet dosages. A standard aspirin tablet contains 325 mg; a common vitamin D supplement contains 25 mg.
  • Microgram (μg or mcg) — 1/1,000 of a milligram, or 1/1,000,000 of a gram. Used for highly potent drugs like thyroid hormones (levothyroxine: 25–200 μg/day), folic acid (400 μg/day), and some vitamins.
Critical danger: mcg vs mg confusion. Confusing micrograms (μg/mcg) with milligrams (mg) introduces a 1,000-fold dosing error. This mistake has caused fatal overdoses in real clinical settings. The symbol μg is often handwritten as "ug" and can be misread as "mg." For this reason, many hospitals require writing "mcg" in full instead of using the μg symbol. Always read labels character by character — never assume.

Volume: Milliliters and Liters

Liquid medications, IV fluids, and injections are measured in milliliters (mL), also written as "ml" (both are correct). One milliliter of water weighs approximately 1 gram. One liter (L) = 1,000 mL.

Syringe markings are in mL (or in units for insulin). A standard 1 mL syringe typically has 0.01 mL graduations. IV drip rates are ordered in mL/hour — for example, "run normal saline at 125 mL/hour."

International Units (IU)

International Units (IU) measure the biological activity or potency of a substance, not its mass. Because different preparations of the same substance can vary in purity, IU provides a standardized way to express how much active compound is present. Common examples:

  • Vitamin D: 400–2,000 IU/day (1 IU of vitamin D = 0.025 μg)
  • Insulin: dosed in IU (100 IU/mL is standard concentration)
  • Vitamin A: 5,000 IU daily reference value
  • Heparin (blood thinner): dosed in IU/kg

Millimoles per Liter (mmol/L)

Millimoles per liter (mmol/L) is used to express the concentration of substances in blood and other body fluids. Blood glucose, cholesterol, electrolytes (sodium, potassium), and many lab values are reported in mmol/L in most countries.

Blood Glucose: mmol/L vs mg/dL

Blood sugar measurement is one of the most common sources of international confusion in medicine. Two units are used globally:

  • mmol/L — used in the UK, Canada, Australia, Europe, and most of the world
  • mg/dL — used in the United States, Japan, and a few other countries
To convert mmol/L → mg/dL: multiply by 18
To convert mg/dL → mmol/L: divide by 18

A normal fasting blood glucose is 3.9–5.6 mmol/L (70–100 mg/dL). A diabetic reading of 180 mg/dL = 10.0 mmol/L. A dangerously low glucose of 2.5 mmol/L = 45 mg/dL.

Travel warning for diabetics: If you manage your diabetes with a glucose meter, check whether your device uses mmol/L or mg/dL. Most meters can be switched between units. Using the wrong unit when interpreting your readings could lead to serious under- or over-treatment with insulin.

Weight-Based vs. Fixed Dosing

Some drugs are prescribed at a fixed dose for all adults — for example, a standard antibiotic at 500 mg twice daily. Others are prescribed on a mg/kg (milligrams per kilogram of body weight) basis, particularly when:

  • The therapeutic window is narrow (too little doesn't work, too much is toxic)
  • The patient is a child
  • The patient has an unusual body size

A common example is ibuprofen for children: 5–10 mg/kg per dose. A child weighing 20 kg would receive 100–200 mg per dose — very different from the adult 400 mg tablet. This is why children's medications always require you to know the child's weight in kilograms, not just their age.

Pediatric vs. Adult dosing: Body weight is the primary factor. A 3-year-old weighing 14 kg has a fundamentally different dose requirement than a 70 kg adult — not just because of size, but because children metabolize drugs differently. Always use pediatric-specific dosing references.

Body Temperature: °C vs °F

Normal human body temperature is 37.0°C (98.6°F). A fever is typically defined as a temperature above 38.0°C (100.4°F).

°C to °F: multiply by 9/5, then add 32
°F to °C: subtract 32, then multiply by 5/9

Example: 39.5°C = (39.5 × 9/5) + 32 = 71.1 + 32 = 103.1°F

The US uses Fahrenheit for body temperature in clinical settings; virtually every other country uses Celsius. If you receive a temperature reading of 38.9 from a US nurse, they mean 38.9°F (normal body temperature), not 38.9°C (a high fever). The number alone is meaningless without the unit.

Quick mental check: If a body temperature reading is between 36 and 42, it's in Celsius. If it's between 97 and 105, it's in Fahrenheit. Numbers outside those ranges suggest a unit error or a very serious condition.

Blood Pressure: mmHg

Blood pressure is measured in millimeters of mercury (mmHg) — a unit inherited from early sphygmomanometers that used a mercury column to measure pressure. Despite mercury being replaced by digital sensors in modern devices, the unit remains universal in medicine worldwide.

A normal blood pressure reading is 120/80 mmHg:

  • Systolic (top number, 120): pressure when the heart beats
  • Diastolic (bottom number, 80): pressure when the heart rests between beats

Hypertension (high blood pressure) is diagnosed at ≥130/80 mmHg. Severe hypertension (hypertensive crisis) is above 180/120 mmHg and requires immediate medical attention.

BMI: Body Mass Index

Body Mass Index (BMI) is a widely used screening tool that estimates body fat based on height and weight. The formula is:

BMI = weight (kg) ÷ height (m)²

Example: 70 kg ÷ (1.75 m)² = 70 ÷ 3.0625 = 22.9 kg/m²

In imperial units, the formula requires a conversion factor:

Height and Weight: US vs. European Systems

In the US, patient charts typically record height in feet and inches (e.g., 5′ 9″) and weight in pounds (lbs). In Europe and most of the world, height is in centimeters and weight in kilograms. This creates conversion challenges when:

  • A patient travels internationally and visits a foreign doctor
  • Medical records are transferred between countries
  • Weight-based drug doses must be calculated from a weight given in pounds
lbs → kg: divide by 2.2046 (or approximately 2.2)
kg → lbs: multiply by 2.2046

inches → cm: multiply by 2.54
cm → inches: divide by 2.54

A patient who weighs 165 lbs is approximately 74.8 kg. A patient who is 5′ 10″ (70 inches) is 177.8 cm tall.

International travelers: Keep a note of your weight in kg and height in cm. These are the values that will be used to calculate drug doses, BMI, and other clinical metrics in any non-US healthcare setting.

Syringes, IV Lines, and Drug Concentrations

Hospital medications are often prepared as solutions with a specific concentration, expressed as mg/mL or μg/mL. Knowing the concentration is essential to calculate the correct volume to administer.

For example, if morphine is available at 10 mg/mL and the prescribed dose is 4 mg, the nurse draws up 0.4 mL. IV medications often run at rates like "dopamine 5 mcg/kg/min" — requiring the nurse to calculate the mL/hour based on the patient's weight and the drug concentration in the bag.

Tips for Patients Traveling Internationally

  • Carry a list of your medications with both the generic name and the dosage in mg (not just the brand name, which varies by country).
  • Know your weight in kilograms — this is what foreign pharmacists and doctors will use for dose calculations.
  • If you're diabetic and use a glucose meter, check whether it displays mmol/L or mg/dL and be prepared to explain this to a doctor abroad.
  • Blood pressure devices in Europe read in mmHg, the same as in the US — this unit is universal.
  • If a prescription lists a temperature for storage (e.g., "store below 25°C"), remember that is 77°F.
  • When buying over-the-counter drugs abroad, don't assume the same tablet strength as at home. A "standard" paracetamol tablet in the UK is 500 mg; in some other countries, 650 mg tablets are common.
